Able Health Services Inc. Targeted Case Management Services will provide participants with the development of person-centered, referrals and related activities, monitoring, and follow-up activities.
Able Health Services Inc. will provide Services to coordinate the support of individuals with developmental disabilities. These services are performed primarily by Coordinators of Community Services (CCS) Our agency will focus on these areas:
Coordination of services for people with disabilities and providing support in the areas that they choose.
Being able to assist participants to have the best quality and most appropriate services and supports within available resources.
CCS will assist participants and their families in implementing participant choice, addressing participant satisfaction, and assuring that participants’ needs and preferences are addressed.
